Care management

What is care management?

Care Management is typically used as an umbrella term for a range of activities intended to improve patient care and reduce the need for medical services by helping patients and caregivers more effectively manage health conditions. With the move toward value based care, Care Management has become the primary means of managing the health of a defined population. It is organized around the precept that appropriate interventions for individuals within a given population will reduce health risks and decrease the cost of care. The Agency for Healthcare and Research Quality defines Care Management as:

“Care management is a promising team-based, patient-centered approach designed to assist patients and their support systems in managing medical conditions more effectively.”

To take this definition even further we’ll add that the goal of Care Management for a population should be “to reduce health risks, achieve an optimal level of wellness and decrease the cost of care through enhanced care coordination and patient insights”. Providers seeking Care Management solutions must consider one that is able to deliver against this model of care.

Summary

The lack of coordination and communication is not only one of the most common consumer complaints about the healthcare system but also a key driver of poor quality and unaffordable care. A major contributor to this problem has been the failure to plan and communicate effectively across settings, especially for care that is being delivered outside of the clinical environment. However, effective care coordination is about more than collaboration and communication; it’s about orchestrating the myriad of tasks that go into delivering the best care to patients, from organizing routine patient care activities to notifying care team members of critical patient events. With Cord, providers now have a simple solution to do just this and provide patients with the best care possible.
